I want to clean this up and make it "pixel art", not

Note I already indexed it to limit the palette. It just doesn't look like PIXEL art.

Hire someone to do it for you.

Make the dithering more uniform, like a pattern. And also introduce more contrast and vary the hue in your values, I think it's called color ramp. I am not really into pixel art but I think one way to separate pixel art from "art made without AA and with a limited palette" is to really think of where each pixel is placed. Like if you move one pixel 1 px to the left it should have a large impact of the entire image.

Try using filters to posterize it in Photoshop. Also, that Mosaic filter.

Then go in, and tweak things by hand.

I've never tried it personally, but I imagine that'd be a way to go about it.
