I don't think you can have a fight or flight response to a picture unless you have some form of a mental handicap. At most a picture can be creepy and unsettling.
Well, you could always send someone hardcore CP with a 12 year old girl ofcourse. That would trigger a fight or flight response and possible cognitive dissonance aswell if it results in a increased blood flow towards the penis of the recipient

Pictures that cause an uncanny valley effect e.g the photo of the rake where everything is dark accept it's white eyes.
The white and inhuman colour of the eyes grabs your attention, the rake looks ALMOST human and that terrifies a lot of people yet it's eyes show that there is something wrong with it.
It is in a hunched attacking stance as if it's coming after you and the brightness of it's eyes forces you to keep your attention on them.
Focus on something terrifying e.g a cute creature being tortured violently in a scenario it doesn't belong in e.g a pony in a warhammer setting, Create a sense of hoplessness that even if they escape from the horror, they have nowhere to go.
