Transexualism can't sort of come as an effect of intolerance against Homosexuals(Ironically)? Since Only dick in Vagina is accepted by society in large and there are some pretty strong gender norms any gay man will have to turn into a women to be able to live out his sexuality. An good example is Iran where homgayuality is bad while it's okay to turn yourself into a woman.
An example of a similar transformation would be have strongly misogynist societies tend to breed many Homosexuals since Vaginas are viewed as disgusting, such as in Ancient Greece or modern day Saudi Arabia.

Transsexualism as in specifically the concept of a person born as one sex identifying as or adapting the clothing, mannerisms, customs and/or social role of the opposite sex within their ethnic group. Though, sometimes transsexuals in history were ascribed their own roles and customs as well, separate from normal men and women.
Two-spirit is a modern term to refer to historical transgender native americans, it was never used before it was coined in the 1990's. There are tons of different native american tribes and cultures, most of them have their own term for transgender people and their own believes about it, the word two-spirit was made to lump them together to make it easier for historians.

There are some homosexual males in Iran that dress as a woman and have genital reconstructive surgery as a way to legally have sex with men, that's not the same thing. Gender identity is different from sexuality, most people who are transsexual feel that way from early childhood and are not sexually motivated.
