He managed to create the Modern state of Russia from Muscovy but he killed his son for some vague reason and was the last of the Rurikids. Why would you kill your only heir after creating one of the principle world powers?

That's what the word terrible means. It retains that sense often, especially in writing. The popular sense derives from that. It's a similar case for awful.

The general idea is Ivan who produces both awe and fear, but there isn't really a single word for that in english.
A worse mistranslation of title is for Ethelred the Unready. The old english word translated as unready: Unræd actually means poorly counseled. It wasn't a title that was attached to him until after his death.
