Because there is neurological evidence to support Gender Dysphoria as a legitimate medical condition whereas the former is just textbook delusion.

Because the easiest way of treating gender dysphoria is trying to diminish the symptoms via making the patient feel more like their percieved gender.
One is acceptable role-play and the other is not. capricious but that is the everyday take-away.
