Thing is it would be hard to actually notice, if people on average die at 40-60 and you look 20 it would be odd but you could blame some bullshit thing you do and then they die and you just hang out with more 20 year olds and say you're 20.

Women who take HRT after menopause keep their youth 9 times out of 10 but most doctors will not let them because of breast cancer.

The drawback is males would have to chemically castrate themselves and change their hormonal balance, grow tits and essentially become androgyne/eunuchs and its dangerous to do so after a certain age (like 30 or so).
