other non-deliberate incidents were a series of radar hiccups and false alarms dotted through the 70's and early 80's.
also the advent of nuclear weapons is mostly seen as a deterrent rather than a tool, no one benefits from winning a war if 1/3rd of your population dies in a 6 hour exchange. Safely to say, there would have to be some extreme situation to warrant a nuclear exchange that'll end up in no one winning.
