Off the top of my head, I remember added scenes such as the whole sequence with the ring Albus found and the village, added lines ("I used to be a mercenary before I took an arrow to the knee" made me cringe hard), unneded changes to some scenes, and painting the main duo in a much more positive light (which I don't mind at all, but an irl friend felt that it was almost fanfic-tier).
An example of the last two would be the duel between the Merc and Dog-face: in the book it doesn't happen, as Merc doesn't go out of his way to save the slaves, and Dog-face loses his fur due to annoying Zero. Overall, in the anime Merc is nobler and Zero less petty.
Take this with a grain of salt as I dropped around episode 5. You can check out the novel at NanoDesu, it's a fun read.

Again, not the artist from the original line of conversation, and I can't speak for everyone, but I'll try and explain my thought process a bit better.
Adding in 'too many' details is an unfortunately tricky business, as details can help many artists figure out what it is you actually want. But at the same time, some prefer more leeway and balk at the restraints 'many details' can add, particularly to a request. Not terribly helpful, is it? (unless you're asking for like, a comic or something stfu this is a free request thing, don't be an ass - unless the artist has said they're cool with that)
Context and scenarios can be a tidy way of adding in details without getting persnickety.
Saying something like 'I want X character to be doing this specific thing with this exact face and also these three other very pointed details' might be off-putting, especially when they're the same details almost Everyone Else is asking for.
If the artist didn't want to draw it the first request, they may resent it the tenth time they see it.
But! Something more like 'X character, being giving a surprise fucking' is a lot open ended, and you can still add some details, like position or outfit, without it following everyone else's formula while additionally giving the artist a little more room to mess around.
I can't swear that this is something that will Most Definitely help your request(s) get drawn, but I think it helps.
TL;DR provide some details, break the formula (especially if you're asking for something a lot of other people have asked for), but leave room for an artist to still do what they want. If they want/need more details for something they Want to draw, they'll probably ask.

I personally enjoy a lot of stuff which I believe is not mainstream to mildly obscure. I'm not actually sure of how obscure they are due to the boards being mostly about recent or cult-classic stuff.
I'll just write down some of the things I'd gladly at least consider drawing but I haven't seen show up since I started doing deliveries:
Seikai (Crest of the Stars) requests, as I was hoping the translation of the newer manga would make some pop up; .hack//, particularly for IMOQ or SIGN; Ar Tonelico, I really love it, but the fabled mishafag likely scared away every other potential requester (I'd like some Luca, for example); Ys series, the eight has come out and nothing shows up.
Other than that, Boktai, Custom Robo, Megaman (Zero and ZX preferably), Metroid, Pandora's Tower, Parasite Eve, Sin and Punishment (Games made by Treasure), Zone of the Enders (fat chance of there being a Rebecca request though), Muv-Luv, Outlaw Star, Patlabor, Nadesico. I'm not getting my hopes up for an Armored Core request, even though I've posted a mecha on human delivery here without troule.
Not an exhaustive list by any means, but just some spotlights. I doubt I'm the only one that appreciates seeing that some niche series hasn't been forgotten. Don't catter to what's popular at the moment, dare to ask for older things.

To summarise:
The creation of new threads brought about the novel idea of using work from the previous thread as candidate for OP pic. Either someone really wanted to get picked or was really happy with their delivery and so went ahead and remade the OP with said work.
The OP post wasn't properly formed, which was my concern, but others were upset as to the decision to pick said image as the OP pic. Better stuff was ignored and no vote occurred, this led to people insulting OP and the pic itself. If the artist/OR was happy about the work before, the amount of crap levelled at him probably didn't leave a positive feeling. It was his own failure for being unable to paste some text in a box sure, but /h/ was extra grumpy that day.
General shitposting aside it's of no consequence to me what gets set as OP so long as basic work is conducted in constructing OP:
Remove irrelevant info, label the OP pic with "Draw T/h/read" or whatever, set the fucking title properly, you get the idea it's not hard.
Anyway yeah, these threads do have consistent issues with requests being ignored, people failing to properly adhere to rules and a general lack of etiquette. Whaddya expect? Discussions are hard, people enter /h/ thinking with their dicks, bad combo.
