By default the game installs to Program Files/Overflow, and the patch wants to install to c:\overflow. I had changed the patch install path to Program Files before but that didn't work. Did it again and now it does. Weird.

Okay, what I said before was about the pre-patched game.

Post patch, it looks like you have to clear the game once and when the mom asks who Makoto's girlfriend is there'll be a third option (which is the mom, assumedly). Keep picking that choice until it no longer lets you. I'm not sure what happens after that, as the game is still messing up (cleared Kiyomi's scene again but then it took me to the gallery for some reason).

Don't know what to tell you guys, as this seems to be the only choice that matters. I got her scene on both Kotonoha and Sekai's routes at the very end, picking random choices at other points.

I got it even after several continues, so it's not that you have to go undefeated.

but when pick the third choice at the start until you cant pick the third choice anymore,then you will have 4 battles in that playthrough.
the 4th battle is the mom which happens after you are done with the third as in nikki.

There needs to be another game in which you can actually pursue the mom's. A major thing that hurt them in Summer/Shiny Days was that they never felt anything more than side characters you could bang.
