Many times I heard opinions that if someone is concerned about privacy at all, they should not use a smartphone at all. Why is that? Can't smartphones be set up properly to be private and secure?

It depends on what level of privacy you're looking for. Either way, the situation is much better than years ago. Now we have F-Droid, LineageOS, Firefox Mobile with support for extensions. Setting a VPN with OpenVPN on Android is easier than on a PC.
However, if the level of security you want/need includes hiding your location from your carrier and/or the government, yeah, no way to fix that. Mobile phones (not only smartphones) need to guess your location to choose the closest tower to handover. Then you can only hope the carrier doesn't log it
