Definitely bad contact between heatsink and cpu. Thermal paste degradation won't be this bad.

Just make sure its real tight
Nothing wrong with making it a bit tighter most of the time
You could try cross tightening it if it loosens on its own (top left then bottom right, then top right and bottom left)
