LAV Filters (Internal to MPC-HC)

If you want xy-vsfilter or XySubFilter it's just a matter of installing them. I have no idea what the differences are.

3.)xysubfilter beta

Thats it. Pick madvr as video renderer and xysubfilter as sub renderer in MPC's settings. Leave madvr at defaults if you have a very weak video card and just tick Debanding/Ringing artifacts, smooth motion, untick everything under trade quality for performance. If you have a strong videocard then chose NGU Anti Alias (medium) under Chroma upscaling and Upscaling.
