most of those are fairly distinctive changes that affect the user a lot, they often bring their own DE or other unique stuff (think of nixos or peppermint with its webapps)

But the first stuff changes the actual Linux kernel and layout completely. The reason android isn't Android + Linux is because it's so different it's barely Linux. It's not even licensed under GPLv2 fully, most of it that's actually notable is licensed under Apache which is NOT copyleft. These kind of differences are all found inside of the separate OSes that stemmed from the Linux kernel.
Debian and the likes are GNU + Linux or GNU/Linux because they do not vary much/at all from the Linux and GNU core philosophies which intrinsically go together well due to their roots.
