This isn't even a design problem with the phone. One of their two battery suppliers who were awarded the contract fucked up a small batch.
Oddly enough the note 7s sold in china are not affected, as they used a different battery supplied. For once it wasn't the Chinese factory that shit the bed.

Most phone manufacturers have gone through a recall process associated with overheating or damage while charging. This goes back to the earliest cel phones.
Apples problems were mostly the adapter being the source of the fire. Years ago my iphone 3 had it's power adapter recalled and in 2015 apple was still having issues with their charger designs and issuing recalls.
Some apple 4c and 5c phones also had battery fires. Apple never offered a recall even after a girls pants caught fire in 2014 from a battery fire. This was one of the rare times a battery fire was not caused while charging.
Unrelated to that there was the ongoing USB type-C "cablegate" where numerous cables would destroy your phone/laptpop or possibly lead to a fire. They originally blamed this on cheap 3rd party cables until the problem was found it shitty OEM cables that were supplied with the original device. For a while Amazon pulled all the cables off their marketplace just in case.
Oneplus and nexus phones were especially plagued with this issue, but virtually all manufacturers were vulnerable because no one makes their own cables.
tl;dr
Everyone has been through this.Even if the design is good the OEM suppliers can fuck up.
Exploding batteries? Maybe you're just holding the phone wrong.

batteries explode, its just he cost of this shit holding such a large power charge, if a battery is damaged, it will get extremely hot, if not catch fire, with lithium polymer being notable because they can explode on charge.
