they don't automatically spy by themselves. rather the NSA backdoors routers, so if there's a target that can be attacked, while detection of said attack is unlikely(value of target/risk of detection), they will do it.
In other words, this isn't like W10 mass espionage, this is about backdooring routers so they can attack people more easily.
