So in EA's Origin there is this "repair game" feature. It is supposed to check and compare all the game's files against the ones stored on their server and redownload anything that doesn't look correct.
My question is, how does this actually work? If, for example, one bit has changed in one file and that is preventing the game from starting, how does this find that difference? It would essentially need to download the original file to be able to compare, wouldn't it? Is it comparing checksums?

Probably.

It uses a hash, just like steam.

Just like steam's, it compares every single file's checksum with server's and redownloads the file.

Not really a stupid question, and he had the answer. He just wanted confirmation.
