is it possible to install spyware that runs outside the OS/before the OS boots and thus cannot monitor? If so, how likely do you think it is, that manufactured hardware already gets shipped in such a rigged state?

It's possible with modifying the BIOS or dicking around with GRUB, but it's hard. An easier solution is to utilize hardware USB dongles to capture keystrokes.
