If water fasting is the most effective way to lose weight, why doesn't someone start a weight loss bootcamp type of business where all they do is lock you in a room for an extended period of time?
Basically you consent to being locked in the room with a bed, water, toilet, basic amenities etc, but no food except vitamin supplements to prevent depletion of micro nutrients.
The reason this would work is because it is easy to start on a diet or water fast, but hard to stay on it once the real hunger hits and your willpower is tested. By having someone else lock you in a room, losing weight wouldn't require any more willpower other than the initial decision to enter the room, because there is no way out until your body has reached its goal weight.

Biggest loser is basically this but with food, but guess what happens after they leave the show? they go back to their shitty habits and slowly gain the weight back.
Even if your supposed weight loss camp offers rehabilitation and diet advice, there's still a huge chance that these fat fucks will gain the weight back since it was so easy to lose it/they can go back and do it again.
The best way to lose weight is to change the entire mindset of the person, make them care about what they eat, track calories and macros, exercise and learn how to be healthy. And all of that has to come from within the person, not given to them. That's the best way to stay healthy and fit for the rest of your life.

Probably a lot of legal issues would come out of starving fatties for a while, even with contracts and shit.
