Could be a number of things, my best guess is shit form. Too many people focus on shifting a heavy weight, and completely ignore their range of muscle movement that they are using.
Following that, I would say shit diet.

I can do weighted pull ups but my back still sucks.. looks maybe even smaller than yours.
I always thought I just wasnt eating enough.
