If it's as simple as 'calories in v calories out', why do people find it more difficult to stay in shape as they age?

TLDR most patients, almost all patients actually, have no idea about their own diet.
The top 1% can tell you WHAT they eat, but literally almost nobody can tell you HOW MUCH they eat.

Older joints and more muscle repair time means pretty much any kind of physical activity is bad for you, except swimming. That is of course, if your cardiovascular system can take it. See, to stay in life, or at least until your retirement. Only a few can say so.

Slower metabolism
Lower testosterone
Joint and back pain
Organs unable to work full capacity
All this together along with the fear of slow/no progress due to old age they jump on magical fads like no other instead of true and proven methods that require effort
If you doubt me look at all the mom science blog or fb groups where they think eating apples or drinking lemon tea will shed pounds upon pounds of grease

The top 1% most diligent at taking care of their bodies at least know /what/ they eat, literally nobody knows /how much/ they eat.

The point stands,you have a base caloric uptake to maintain your body, and yes it does differ from person to person
Lower metabolism lowers it, less muscle mass lowers it, joint and organ damage mean less intense workouts
But calories in-out still stands, its just that people with higher metabolism and more muscle mass have a bigger base to work it (needing to eat more as a result)
Just like Michael Phelps at his world class shape need to eat 10k calories while training to maintain his body, but a 80 years old wheelchair lady will likely explode eating the same amount

Caloric intake goes up due to improving levels of income and social gatherings centered solely around eating and drinking.
Most people aren't even better when they're young, it's just their job/metabolism let's them get away, kind of. I'm 30 and most of my friends are already growing a gut and spouting shit like "Of course, I also had abs and could run when I was 20". On the other hand, the dad of a friend of mine is 63, mountain bikes three times a week, watches his diet, dude is trim and lean.Losing weight after 50/60 is hard, but maintaining a lean body is definitely doable.
tl;dr: fat old people are lazy over-indulging fucks who got away with it until now.
