I'm trying so hard to maintain proper form but I feel it slip sometimes. I don't want to die.
so don't up the weight?
if you can't maintain the form on anything but a PR, then you clearly can't do that weight and need to scale down.
Have you tried recording yourself and comparing yourself to videos of other people? Asked any local powerlifters? Asked in the /plg/ thread?
It's really the only way you'll get any wiser.
