need a side shot, but from here looks like you need to sit down into it a little more, your hips are shooting up before you lift the bar off the ground.
Lean back a bit and make sure the bar is sitting at about midfoot also make sure the bar is aligned with the shoulders. this should make your shins run more perpendicular to the floor so you won't be moving the bar past your knees.

Also, just a general question, how long is too long to take between reps in a set of a deadlift? I feel like towards reps 4 & 5 of each set I start to take ~5-10 seconds between the lift to recompose and get ready for the next pull. Does this mean I should lower the weight?
