Rows especially. Too much chest tension and weak back muscles draws your shoulders forward. Make sure you focus on keeping yourself straight during the movements

It was already said twice but I'm going to put in my own anecdote. I added face pulls to my routine and the next time I saw my doctor I got complimented like crazy about how much my posture improved.
