Why do you use treadmills when you can just run in the real world where the view is nicer and less repetitive? Does it make any difference at all?

Exactly. Treadmills are actually harder on your legs. If you're going to use a machine once in a while, use an elliptical to work done different muscles and reduce impact on your bones and joints.
Using treadmills is absolute hobby-jogger/dyer status.
