it's possible, just it takes effort. if your friend wants to lose weight, now's the time to start eating healthier. if he wants to gain muscle, now's also the time. now's the time for him to look around for new haircuts, start shaving (with an actual blade), looking for new clothes (well, i'd personally wait to buy stuff until he's 16, because that's about the time guys stop growing), and pick up good social habits (this is also assuming he has good hygiene already. if he doesn't, then he really needs to start).

basically, as long as he doesn't have an eye where his nose should be, and his mouth isn't on his chin, then he should be able to look good.
