This is a good thing you're doing, OP.
I'd start by buying 2-3 controllers and taking one apart. Check how everything fits together and which wires go where. Then, just create the input system you want (if XYBA will be his foot, then four switches) and link it to the place where the thing you're replacing goes (wire from the foot switches to the PCB or whatever, i know nothing about xbox controllers)
You'll probably need to do some soldering. The extra controllers will be useful for parts. Other than that, ask specific questions in electronics threads

Gyros would be neat. Orienting the controller would be right analog. Or mechanically, have the foot be right analog.

Too bad he lost his right hand and not left. If he could use his right hand, he could game on a PC as mouse+keyboard with a MMORPG mouse with 12 thumb buttons, those 12 buttons being assigned to WASD and other keys. Razor and Logitech make them. Maybe you can contact them to see if they have left-handed models, esp for handicapped gamers.

If you have a lot of money to spend on experimentation, a 3D space mouse can completely replace both your keyboard and mouse.

The question is if you're willing to put a lot of money down on what might not be the most ergonomic solution (for gaming)

/diy/ is the right place for you to ask.
The other boards suggested are all retarded when it comes to this stuff.

Youre going to want to relocate the right side buttons to the left and then house it in a custom built case. Get decent with a soldering iron. Get some solder wick, it will help remove the solderd components.
Get some perf board to resolder them too. The xyba buttons are not relocatable, but you can scrape the black stuff off or scrape the paint from the traces and solder to them, just add some wires and buy some buttons to add to the perf board.
The rb button and the trigger are both pressure sensitive, though I don't know anything that uses the rb's sensitivity.
The way the trigger works on the one controller is by sensing the magnetic field. IIRC the triggers are normally open, but you might want to double check that.

What if you added something on the floor to replace one of the analog sticks? Maybe something where he pivots on his heel. Toes up and down for backwards and forwards, left and right in kind of an arc. Terrible drawing related.

Look into the AbleGamers charity. They'll be able to guide you in the right direction and they even have grants you can apply for that will cover the cost of equipment like eye trackers and tongue joysticks

What system are you working on? If it is a 360 I'd be willing to hack into my controller and see if I can set it up for you, you'd need to make a foot control box somehow though, if that's what you're sticking with.

Use arcade buttons for a foot controller
Hell just use a whole fighting stick setup, he can hit the stick for dpad movements,

Remember, you can rebind all the buttons on an xbox one controller if you need to
