As far as I can tell, no. Bombs/hearts/other bullets pop out of the girls and you shoot them, and the goal seems to be "shoot girl until meter is full".
The most disappointing part is that it looks like there is no interaction possible at all. If they see you or hear you it's an instant game over (not even a cutscene), which means there is little chance you can climb the girls or anything. The problem is that the people who churn out that kind of garbage are usually the same who can't be bothered to spend any time watching tutorials or improving their technique. It's not like such tutorials in the basics of MMD (and animation in general) don't already exist.

The same is true everywhere. "Illustrators" who don't want to spend time improving their understanding of anatomy because "but muh style!"; "collagers" who don't want to spend the extra few minutes finding images which match lighting/perspective, or who don't want to bother cutting images out properly; "writers" who don't want to bother to go back over what they've written to edit, etc, etc.

The issue these days is almost never a lack of good educational resources, it's with laziness. It's trivial to find people who will gush over even the shittiest piece of work, so if people are only doing it for praise, there's little incentive to improve.

"Suddenly"? Reception of MMD size stuff years back was godawful compared to today.
It's still pretty bad, mind you, but that's kinda fair. Honestly, it really is the most creatively crippling and kinda boring of all common mediums for size content. Hard to fuck up yet low skill ceiling, there's a reason most people use it as a "compromise" and that the best MMD creators i.e Manzi are good at other mediums too.

Sure, but then virtually anything can be used as a creative medium if you're dedicated enough. Some platforms more suited for it than others though, and when compared to things like illustrations, writing and even actual 3DCG modeling, just making pics and videos with MMD really is the most inherently limiting and gray option, which balances out with how it's a lot easier to dive into and start creating the stuff in your mind with no previous knowledge.

I saw it the other day, it's a decent movie in its own right, but it has very minimal fetish appeal.

The problem of power tripping/being corrupted by the power the monster gives is handled fairly well, though, both as its own theme and as part of an allegory for an abusive relationship. So if you're interested in that side of /size/ writing or just want to watch something a little different I'd say it's worth seeing.
