Eh, it would be a pretty quick death. Sure the idea that it's a burning hot dick makes it seem worse, but unless you're an especially resilient super it isn't going to last long enough for you to really suffer. Not much different from having your brains blown out really. Unless you're one of those supers that comes back as undead and you somehow remember things that happen just after you died. That would really be shitty.

Everything about him screams way too edgy to take seriously, but man Warren does a great job of making this fucker actually scary. Hell, he does a great job of giving an eerie dread to a lot of the things in the setting.

I have never seen any indication that Warren digs guro.

His storytelling is dark (I refer you to the old bubblegum Crisis stuff he did) but he's never gratuitous.
