Doom CAN rule the world, but a world he rules will never be sustainable past his own will to power because he doesn't have priorities that underlay a world anyone would want to live in.

He could absolutely turn the Earth into a factory for spaceships to go out an conquer the universe to make more spaceships to conquer other universes, but he wants to make a world that will love and accept him and because of that he cannot create either.

He can do anything except force people to love him and therefore will ultimately fail because it's the only thing beyond his grasp. The only person who ever loved him was his mother and he tricked her into hating him so that her soul could be released from Hell.

I like to think Latveria is actually good, for the citizens in some respects. It's safe, crime is low, everyone gets top notch health care and education.
Dissidents are handled roughly though, same as criminals. Way too roughly.
But there aren't that many dissidents because of three reasons

2)fear
3)genuine good lives in latveria if you're not a dissident

But the 3rd reason actually matters and post 1st decade of life under doom it's the biggest of the 3, for most people.

He used to deal with coup attempts all the time. Once Latveria started to have more modern infrastructure. He also has he advantage of ever present enemies that he has somewhat earned the loyalty of the people via defending against them with his army of doombots. I'm assuming you may have read Secret Wars and not the Avengers/NA run or the FF. Doom didn't "Create" the Battleworld universe. as the incursions went on Both the Cabal and the Illuminati noticed that parts of the worlds being destroyed were actually surviving. Doom pieced theses disparate parts together. Almost the entirety of his power is focused on this alone. He's holding it together and afraid to let go (unlike reed at the end who isn't afraid) that is sort of the whole fucking point of the story youngling. Gaining omnipotence really fucks you up (see: Secret Wars 1984, Infinity Gauntlet etc. etc.) and you can't lose focus for an instance. Battleworld is ultimately a failure, but without it there would be nothing left because Reed wouldn't have anything left to take the chance with. The worst thing you can say is that Doom loved being God too much to try and recreate the multiverse and he was punished accordingly (Face wouldn't heal, Forced to kill strange, abandoned by Thors, Susan and Valeria, Fucked up by Reed) but even Reed saw the good he did and gave him a happy ending. Franklin is a "Universal Shaper" trained to create universes by his future self who at that time was a fully powered Cosmic entity with a pet Galactus and around 1500 years old who was actually strong enough to survive in the vaccum of the Death of everything. Franklin also has a support staff of the smartest people in the universe and a properly functioning (multiple) Molecule Man instead of the deranged (mono) molecule man.

tl;dr : Doom liked playing God too much but he was very very good at it
