Those people haven't since interacted with anyone else even each other, so how can we be sure he wasn't just thinking about possible outcomes of his past actions? As Ashi became purer and more filled with hope, she pulled Jack up with her. She represents his inner state, so it wouldn't be that strange for her to be a figment of his imagination.

Jack is actually a normal 2010s man who got stabbed by one of the samurai swords in his collection and had to be put in a medically induced coma. He's not even asian. In the coma world he imagines himself as a samurai hero like he's in one of his favorite films. Ashi is the nurse and Aku is his shitty cousin who keeps trying to pull the plug on him.
