It's complicated. Skeletor while in Hordak's service he tried to overthrow him, by using dark magic, which transformed him to a skull ram demon. But he failed and Hordak slay him, and took used his skull to create the Havoc Staff. When he offered the Staff to a younger Skeletor, the contact with the Staff, revealed at Skeletor, what eventually will happen to him, and he used the Staff to defeat temporarily Hordak and escaped with the Staff.
This is the Havoc Staff's origin from the Eternity War. Though there is another origin from the modern mini comics, a bit less complex.

In the mini comics the origin of the Havoc Staff is closer to the 2002 cartoon. The Staff was one of the powerful objects, able to channel the Star Seed's power, source of every magical power on Eternia. Hordak adviced his yound apprentice, Prince Keldor (before he became Skeletor), to take it from its rightful owner, a powerful sorcerer named the Faceless One, Lord of Zalesia.

In Zalesia, Keldor engaged the Faceless One in battle, but when the Faceless One, who is Evil-Lyn's father, had a vision, that the young man that he was fighting was his future son in law, and that through Skeletor and Evil-Lyn's son, his bloodline would continue, he gave the Havoc Staff to Keldor.

In one of the He Man comics, it turns out the staff is Future Skeletor who was a minion of Hordak and gained power and tried to overthrow Hordak, bit was defeated.
Seeing this cycle, Skeletor somehow broke free of this loop and did not become his own staff.

The early 80's have been the Sword & Sorcery Gold Era. There have been at least 3 movie franchises (Conan, Beastmaster, Ator), and 4 or 5 animated ones based on barbarian characters (He-Man, Blackstar, Thundarr, Lion-O, Fire and Ice). After 1985, their popularity reduced, and MOTU followed them.

She is powerful witch. She was a beautiful woman once, but Hordak convinced her to sacrifice her beauty and her unborn child, for gaining more powers, and she transformed to Shadow Weaver. Her powers are magical ones, so basically include teleportation, levitation, hypnotizing, magical force fields, stuff like that.
