I think the model that anon is visualizing is one where the timeline already exists in its entirety and the past and the future are happening simultaneously but being that we're human and we're watching/reading this stuff from the POV of other humans it's just a matter of perspective.

He didn't. He got lucky that Sunny made FoxALIVE and Naomi thought to take it with her. Without it he was fucked.

He also died thinking he created a new wild west/Outer Heaven and severed governement/society from networks, but didn't.

Even his hero Big Boss came back for two minutes to say "Man Outer Heaven was a dumb idea what in the fuck was I thinking." Ocelot fucked up because his gayness for Big Boss didn't let him see the big picture and he acted too soon.

In case you are wondering, this is from Hulks: Agents of Smash, this is in a bad future, and the twins you see there are confirmed to be She Hulks kids. The green fellow is actually The Leaders kids. But back to the twins, you know who the dad is?

Well, as it turns out, at the end of the episode, Hulk asks Red Hulk, who is really General Ross, if certain traits are hereditary, and he says yes all suspicious like, and Hulk just smiles.

TLDR, General Ross fucked Bruce's cousin and had twins.

No, he's revealed to be an orphan with no memory who just showed up there one day. Also, this was in the negative zone, not just another planet, so explaining Hulk had another kid in a whole other dimension that he never knew about would take some doing.
