Because they can be. In some states it's legal for them to negotiate with your insurance, but then charge you again for anything your insurance doesn't pay. So if your insurance only covers a $4k ambulance ride, the ambulance company will charge you the remaining $2k. Most other medical programs don't work that way, but emergency responders are nearly untouchable in politics and any politician who tries to regulate them is going to be buried in attack ads about killing children and inhibiting emergency services.

Most people don't even realize ambulance companies are private, so they assume the state is trying to punish under-funded civil servants. The average American believes ambulances are state-run because the alternative sounds ludicrous in their heads. But that's how it is.

The US almost always votes the incumbent to stay for a second term. So far the Trump victory is still following normal political trends at least as far as the pendulum swinging goes. A lot of the most avid voters don't really care who the candidate is. They just look at whether he's red or blue.

Plenty of analysts are anticipating major corruption scandals in the coming years, and the signs are already there pointing to it. Lots of people getting installed into places you may not think they should be. Regulators being hired to regulate industries they still work for. That kind of thing.
