Because they can be. In some states it's legal for them to negotiate with your insurance, but then charge you again for anything your insurance doesn't pay. So if your insurance only covers a $4k ambulance ride, the ambulance company will charge you the remaining $2k. Most other medical programs don't work that way, but emergency responders are nearly untouchable in politics and any politician who tries to regulate them is going to be buried in attack ads about killing children and inhibiting emergency services.

Most people don't even realize ambulance companies are private, so they assume the state is trying to punish under-funded civil servants. The average American believes ambulances are state-run because the alternative sounds ludicrous in their heads. But that's how it is.

You know, part of the problem was they went out of their way to villainize him and his voters to the point where it ceased to be believable. They couldn't merely rest on Trump's checkered past and corporatist nature, or otherwise people would have asked what the difference was from Hillary.

Plenty of analysts are anticipating major corruption scandals in the coming years, and the signs are already there pointing to it. Lots of people getting installed into places you may not think they should be. Regulators being hired to regulate industries they still work for. That kind of thing.

Trump is like the warning bell for when a country's political system gets too dirty. Eventually blatantly unsavory characters can get in by gaming the system, and once that happens it only keeps getting worse.

If the Republicans hadn't repealed the limitations on donation money, we likely wouldn't have Trump in office now, but they wanted all that cash in their pockets, and the resultant overflow of competition and public disgust made most mainstream Republican candidates unelectable.
