Because in the west, there's little distinction between "gay stuff for men" and "gay stuff for women", when the two demographics have little in common from a consumer stand point.

Remember, Japan is a lot more traditional with gender roles. Yaoi is a way to write romance while avoiding dealing head on with the dominance/submission of a traditional hetero relationship, which may put off inexperienced women when they're looking for schlick material. Less rigidly defined relationship roles in the West mean less demand for yaoi on the market.

There's the strong forbidden love angle too (Japan is still v. homophobic in some ways), and the archetype of the bishonen in Japanese media goes all the way back to feudal times.

Also, ignoring fandom, the Western equivalent is in novels. Look up Captive Prince.
