It does bug me that out of all the people in either Avatar series, only that one Earth Kingdom general in the season 2 premier thought of sinking someone's feet into the ground, instantly incapacitating them. If they had ever tried that again, any of the villains could be instantly stopped.

..Two things:
First, earthbending in the show is different from simply moving rocks. It looks like the rocks they earthbend are 'put together', which makes them more breakable. It's noticeable that every time people are hit with earthbending, the rock breaks. People have survived stuff that would cause very serious injuries like that. It's like they are simply putting earth together with their energy, but as soon as it hits, it breaks (like waterbending, actually. When people hit the water and break the connection, the 'power' stops and it becomes regular water)
Second, never consider that power is absolute. Tecnically a waterbender could turn you inside out. They don't do it simply because they don't have the power to do so. So you're saying an earthbender could just impale people with spikes, but we don't see them doing it simply because they can't, or because it would take too much time and energy.
That said, there are war crimes in ATLA that are not mentioned in the show, that could be it.

According to Bryke, this - and things like Zarheer taking the air of people's lungs -is against the "rules of war", its like using weapons banned by ONU or attacking medics in war. Its a sign of dishonor and might cause problems to you.
Iroh, according to Bryke and some drawings that they made, had this on Iron backstory. He manages to get one of the Walls of Ba Sing Se, until a capitain of earth kingdom panicks, and get a lot of people to violate the rules of war: making a huge hole with an earthquake and closing it, destroying Iroh's entire squad, as well as killing his son.

Fire being tied to electricity means you could challenge both energies and force someone to suffer spontaneously combustion from a distance.
All 4 elements are broken as fuck and all are readily available to a true master of the art.
That said air and fire bending have a nearly infinite offering to any skill level where earth and water elements are both more limited as easily seen.

Nah, not even that, they don't technically have ever historically existed. The geneva convention will go out the window the moment a serious war between superpowers will break out. Rules of war are just propaganda that the winner accuses the loser of violating therefore giving them the divine right to win.
