Yup, in most flashbacks to the early 20th century and WWII he looks exactly the same as he does now. Though I dig the older look for him, it seems to fit him better.

Depends on the Universe. In the Comics his Origin Story states he was born in the 1880s, but he seemed to stop aging when he hit his prime. Some other comics have hinted that this may not be true and he may be in fact older. But those plots were forgotten several years ago, and now that the 616 version is dead will be forgotten indefinitely.

I think Millar stated that he aged so noticeably because he stopped acting like a berserker. It caused his healing factor to slow down or something. That's why he gets fucked up so badly in the comic. I think in the original plan for the sequel he was supposed to start de-aging.

Hulk: The End and Planet Hulk are two fairly good ones which focus on Hulk's character. The first is a one-shot while the second is a larger story arc, more along the lines of Old Man Logan. I wouldn't bother with the follow-up to Planet Hulk (World War Hulk) mainly because it's a bit silly and fanservicey with Hulk beating everyone up, and because the whole Planet Hulk story tends to wrap itself up nicely and you don't necessarily NEED something to follow it. But it's there if you are curious.
