Because insane or not, Batman would understand the difference between a psychologist, psychiatrist and psychoanalyst.

Only one of those is capable of diagnosing you. If Harley's a psychologist, she's not legally permitted to make diagnoses.

If she's a psychiatrist, that means she already went through med school (5+ years), then another probably 5 years of training on specialist wards in psychiatry. You can't start med school until you reach the age of majority (18 in the US) because of the drugs and other chemicals you'll be expected to handle, and because as a minor another person cannot legally be your ward - ie, you can't be made their doctor, which is an important part of training new doctors. I know, it happened on Doogie Howser, but that was made up. In real life nobody would ever be permitted to attend a medical school in the US below the age of majority, let alone graduate - and you can't cut the time to graduation down either, because an awful lot of it is based on practical work, hours on wards under supervision, that kind of thing; not on exams. On top of that, you have to be registered and licensed to practice medicine at the state level, and anybody who isn't is legally not capable of diagnosing a turd they stepped in. Batman wouldn't see any reason to listen.

So Harley would need to be at a minimum (assuming she didn't flunk out of her psychiatry training - she'd still be an MD, which is the lowest possible medical degree and equivalent to the Bachelor of Science awarded in other countries) 28 years old when she encounters Batman as a rogue. Does she look like she's pushing 30? She usually looks like she's pushing 20.

tl;dr because she's not capable of diagnosing him and likely has an undergraduate degree from a liberal arts college. Yes, psychology is a liberal art.

Plenty of doctors,scientists, engineers, etc. in the real world are retarded. Being book smart doesn't translate into being intelligent in other areas automatically. I was thinking of just one case where a research team faked having cloned a human and did a terrible job at faking it, at that. Plenty of other cases where someone with a fancy degree from a good school did something mind-numbingly stupid pop up in the news.
