I think it's more or less in the proportions of the ingredients, rather than there being a secret ingredient since colas taste similar. We know they use cola nut and coca leaf extract though.

These days it would be trivial for a major competitor to reverse-engineer it. A lab using a mass spectrometer could easily figure out what the ingredients were, and in what proportions. That would be impractical for someone making soda at home but it would be no big deal at all for a corporation with deep pockets.
