I don't know how trezor works but you need an ethereum address that you have the private key to. If you have the private key then you are good

Just send the ETH to whatever address the ICO is and they will send you their ICO token back. Or, if you find that someone is doing it via a service, give them your public and private key and they will transact that way. Simple really.

Yes you can, you would send the ETH to the smart contract address, and you would receive the token to the ETH address. Sending the actual token would be a little more complex. I use EtherWall and it has Trezors support I believe, but in there you would add the contract address and from there you can communicate with the contract to send tokens or whatever. You can also check on etherscan for your tokens.
