Doc here..Looks more like erysipelas to me, note the distinct raised margins and more widespread involvment. Septic arthritis typically presents with a more focal swelling over the SC joint. But obviously its near impossible to diagnose from just a pic so definitely needs to get to hospital. Otherwise this is just speculation.
