I have thought about it. The problem with the US is passing USMLE. It's a tough exam as a lot of it doesn't relate to the actual day job and the first part is essentially a biology/chemistry science paper which I have long since forgotten after leaving medical school. You then have to score highly as a non-US applicant. It's basically setup to prevent foreign doctors coming to the US and lowering salaries... the AMA is a very powerful union and has really helped US doctors maintain a monopoly.

Canada I have thought about more seriously as their entrance exam (MCCEE) is a lot easier to sit and salaries are still good with decent quality of life.

I've also thought a lot about New Zealand. There are apparently good salaries going in Dubai/UAE but that isn't really my sort of thing.
