In some countries they have/do divert confiscated illegal drugs such as heroin for use in hospitals and such, if it's of high enough purity.
The main reason they wouldn't use heroin as a method of capital punishment is that it isn't as easy to overdose on heroin as people think, with widely varying tolerances amongst individuals. Also, a prisoner could prepare themselves with something like naloxone which would make the heroin pretty much useless.
