It certainly will almost fully turn off sperm production on even a low dose, but permanent damage doesn't occur for at least a few years, even on a standard dose of spironolactone and estradiol.

If you suddenly stop taking your pills, you will find a pretty large difference in moods, gaining more testosterone-generated feelings, and you will start generating sperm again. The sperm production takes a few months to return back to a level similar to pre-HRT, but it really does come back.

There is not a large risk of permanent or irreversible damage from HRT, other than breast and butt fat tissue lasting for years after stopping HRT. Every male attribute will come back given at least a few months.

It is interesting how the decrease in strength happens. On a low dose, I would say it was, at most, 10 or 15%. However, it seemed to occur at the extremes of exertion. Like, you wouldn't notice the decrease in strength unless you tried to pick up a 150 pound box. Prior to HRT, it would definitely be difficult, but HRT made it pretty noticeable. Even today, I don't detect much of a decrease in strength when it comes to endurance-related activities, only gorilla lifting.

There's probably some truth to that, though there are quite a lot of women with long faces, too. But if you're an extreme case of manly, there's always surgery to fix it if you really think it will clock you as a guy after HRT. All are under a couple thousand bucks each if you have decent insurance.

The largest giveaway that you're a guy is facial hair, acne, and eyebrows. You'd be surprised how little even an Adam's Apple matters once HRT really kicks in on your face.
