Training at major trauma centers is hella overrated for this very reason if you're in EM. You're going to leave the ivory tower and go to the community and you won't have trauma surgery to swoop in and clamshell for you. Try to get your hands on as many of these rare procedures as possible. Careers are long. You're going to cric somebody some day.

that's good advice the other doc gave you. i always have 1st, 2nd, and 3rd available, if not laid out. tube and bougie, rescue and bag, blade. but if there's a good reason why 1 doesn't work, i would go straight to 3.

it's not a bad idea to get tied in to the military - i assume you're american. they'd probably be happy to let you train with them. doing live tissue airways on pigs can give you some training scars, but holy shit it'll make the procedure a lot more familiar. a lot of their special forces medics will have more experience putting in crics than your average traumatologist or anesth will in a career. something to consider.

#1 For you all you keyboard cowboys out there, cric'ing a child is not a walk in the park. Adults have a couple of minutes of oxygen reserves after respiratory arrest. Kids don't. They go from respiratory arrest -> cardiac arrest -> brain death quickly.

And if you're pulling the knife on a child, they've already failed other airway attempts, so chances of him walking out of the hospital are low.

#2 I've been in many, many bad airway situations. Too many to remember. There's a big difference between trach'ing an intubated patient or cric'ing a pig/cadaver, and cric'ing a blue child in front of you.

his initial option was to try to jam a tube into the trachea through the mouth. because of all the swelling in the area, this was very hard. he tried to use a small flexible stick into the airway as a guide, but that ate up time, and might have worsened the swelling. the brain and heart are slowly dying while this happens.

he then decided to cut straight into the throat through the neck to bypass the swelling - a cricothyroidotomy.

Here's how it went down.

Guy ODs and his friends drop him off in front of the ER at around 6 in the morning. He's pronounced dead.

He is placed in the walk in cooler at 8am.

I get him at around 11am.

He's unusually warm. It just don't seem right.

I call in the pathologist. I ask him to check if he's still alive. He gets a stethoscope from his office and comes back and checks.


He says " No, "lol" , he's dead."

I proceeded to do the "Y" incision.

He bleeds too much. Usually there is little blood.

I kinda freak, I call the pathologist to check him again.

He does, and says proceed.

By this time, the pathologist thinks I'm crazy.

He leaves the suite.

I proceed, I keep going. I get out the hedge cutters and remove his chest plate.

I cut open the parcardium, and I remove his heart.

Blood fills up his chest cavity so fast. Like it seemed to be at least a gallon or two of blood.

There was alot of blood and it was warm

ICU fellow here. Cric is an emergency procedure through the cricothyroid membrane which is higher up and easier to do but with higher risk. Tracheostomy is lower down and is for more long-term ventilation when you have time to do a proper procedure.

The thyroid doesn't actually sit that high up. If you do it percutaneously you usually go through the 1st or 2nd tracheal rings where there is no thyroid. If open then you retract the isthmus or divide it before inserting. There are case reports of going through the isthmus and there were no complications.

By the time you found out his airway wasn't competent he'd have had massive brain damage anyhow. Even if you'd cut and tubed him a couple minutes earlier he'd have been full potato.
