It depends how incompetent the website is, there was this was this one site that was meant to be a database of "online bigots" The "upload image" page unintentionally allowed users to upload anything, even html which the site would then implement. So of course someone uploaded html which replaced the whole site with an image of an anti-Semitic caricature.
