depends on the person. I never say shit I don't mean, but I've also known people will say the most psycho shit when they are angry and they never mean any of it. I can't be friends with people like that.
When I'm arguing with someone I deliberately say shit I know will hurt them. Breaking them down from the inside sort of thing. I don't mean it but if someone pissed me off is can be a nasty bastard.

To make them feel bad, it's a tool to say really mean things to them so they feel bad to make you feel good/better because that person is either pissing you off really badly or trying to inflict same damage on you
