Health issues due to being a runt? Not always, sometimes they are perfectly healthy. And just being the smallest one in a litter doesn't always make it a runt. Runts are smaller than the average animal of that age, so even the smallest kitten of a litter may still be average size as a whole. Generally an unhealthy runt is seen right at birth; they have trouble breathing, eating, become ill, and later on develop slower. If none of that happened you're probably fine.
Your cat will get health problems later in life though.

one of my cats was (unintentionally) a product of inbreeding, and is also the runt of said litter. my cousins took in a stray that ended up being pregnant, they kept a male from the litter but didn't get either fixed in a short enough order. that being said, this cat is the biggest pain in my ass. around the age of 4 or 5, he had to get a surgery on his dick so that he could pee without getting crystals stuck. the surgery was the only option after he blocked up 3x after having a catheter inserted/removed. he has a malformed bowel which requires high fiber foods. not just any high fiber foods, but prescription ones, because whatever shit they put in them works so much better (royal canin is such shit food) but at least it saves us from going to the vet for him to get an enema when constipated. oh, and we also give him miralax daily. none of this is to say your cat specifically will have all kinds of issues. some non-runts have it way worse off and some runts have 0 problems. just be ready for if/when something happens.

My cat Lucy was the runt of her litter. Her mother actually abandoned her; she'd nurse the rest of her litter but not Lucy. So I adopted her and bottle-fed her.
She's about 14 now and her only health issue is a heart murmur. Dunno if that has anything to due with her small size.
