The asshole's muscle was moving for some reason while you photographed it. My cat does it sometimes.
If it is like that permanently, then it means that the cat has a prolapse, probably for viral gastroenteritis. It can get worse, it might be contagious for other cats and lead to a horrible death of the cat.

You're not supposed to take in random cats off the street. Presumably it's a stray instead of a feral cat, since it isn't attacking you. How do you know it has no owner? Or that it isn't sick? Take it to a vet.
