Idk, I just remember getting physicals in high school for sports and the doc would put his hand in my pants, grab my sac and tell me to cough, and it was really uncomfortable having his hand in my pants. I haven't had a physical since hs and a clinician recommended I get one when I saw her for an ear infection but I'm anxious about this (if you couldn't already guess, I'm a virgin and not comfortable with people inspecting my genitals)

What the doctor did was to inspect the development of your genitals and if there was any pathological sign (wider than normal canal).

If you have an ear infection, the doctor will check your ear, maybe listen to your chest during a very thorough examination

Yes you can just ask them to skip any testicular examinations, they're there to help. I would suck it up though because you might miss something important like testicular cancer if you check by yourself.
