I have just been asked to give a reading in church for a wedding

Prepare, is there a script or are you supposed to write something? Rehearse, speak out like you would in church, first by yourself then preferably in front of a friend.

A week is plenty. Also remember that your heart palpitations, sweating and general anxiety isn't as obvious to others as it is to you. Also people probably care a lot less about your speach than you imagine.

Simply speak privately with the person who asked you and say you are very nervous speaking in public and would prefer not to do it.

Plenty of people dislike public speaking and, without making a big deal of it, someone else will be found to do it.
